The Wonderland that you knew from childhood has gone horribly wrong ... again. This is the follow-up to designer American McGee's twisted take on the classic Lewis Carroll tale. Designed with the same dark style of the original, but now featuring entirely new visuals, story and game design, Alice: Madness Returns follows Alice on a journey through a wildly corrupted and shattered Wonderland to uncover the truth behind her haunted past and tortured psyche.

Alice: Madness Returns takes place 10 years after the conclusion of the original game, with Alice struggling to recover from the emotional trauma of losing her entire family in a fatal fire. After spending a decade institutionalized in an insane asylum, she is finally released to the care of a psychiatrist who just may be able to help her conquer the nightmarish hallucinations that still haunt her. Alice embarks on a mission to root out the true cause of her family's mysterious death, jumping from a gloomy and stark London to a rich and provocative Wonderland.

Madness Returns takes place eleven years after the events of its predecessor, the same amount of time between the two games' releases. Alice was released from Rutledge's Insane Asylum at the end of the original game, and now lives in Victorian London under the care of a psychiatrist, where the traumatic memories of her parents' deaths in a fire continue to haunt her. Although she seemed stable for a decade, she now begins to experience previously repressed memories that indicate that the fire which claimed her parents' lives (which in the original game was seen to have been caused by a cat knocking over an oil lamp near the fireplace) may not have been as accidental as she remembered; these memories will take the form of retcons to the existing backstory. However, the stress caused by regaining these lost memories has caused her hallucinations and internal struggle to increase in severity, and she returns to Wonderland in hopes of security and comfort. Unfortunately, it has once again become warped by her growing insanity, and she must save its residents - and herself - from the evil that is taking over Wonderland and also embarks on finding the true cause of her family's mysterious deaths if she wishes to retain her precious sanity.

The third part opens with the Nameless Hero and his friends sailing to a new continent overrun with orcs, arriving in the Myrtana, the central region of the continent. The hero lost all his stuff from the previous game when his ship is stolen while he's onshore with Milten, Diego, Gorn, and Lester. Presumably this is the source of the orc invasion that was launched on Khorinis during the second chapter. These lands have no physical connection to Khorinis or the ruins of the penal colony. In these mountainous forests the orcs have enslaved the human kingdom with only a few free humans living in the nearly uninhabitable icy northlands of Nordmar and the southern desert of Varant. The hero must decide whether to join the rebellion and stay true to the deposed human king, serve the Orcish usurpers in their quest to topple the last remaining human stronghold, or choose a path that serves his own ends. Throughout the story, he is accompanied by a number of NPCs, some of whom are old friends. While this chapter brings forward friends from the previous title (Xardas, Diego, Milten, Gorn, Lester, Lee, and Vatras) it also introduces two new major characters; King Rhobar the Second (who ultimately was responsible for sending the Nameless Hero to the penal colony in the first game) and Zuben. While the king has a strong past as a bold leader, he now faces a near defeat; his fame on the decline. Zuben leads the Hashishin that inhabit the southern region of Varant.
